Book an appointment online or call 705-325-1311 to arrange for an appointment and to confirm that your document can be commissioned at City Hall.
The City will commission documents such as:
- Vehicle transfers (Ontario forms only)
- Province of Ontario government printed forms – affidavits, oaths and statutory declarations
- Bank documents
- Legal name change forms
- OSAP – statement of common law status
- Foreign pension documents (no charge)
The City will not commission documents such as:
- Estate, Will or Power of Attorney documents
- Real Estate documents
- Documents that are leaving the country
- Consent to travel letter
- Immigration applications
- ‘In lieu of guarantor’ documents
- Documents that require a Notary Public
There is a $41.00 fee per document.
Original Government Issued Photo Identification is required for the Declarant (the person signing and taking the Oath). The document being commissioned should be completed prior to arrival but NOT signed by the Declarant until they are in front of the Commissioner. If the form requires witnesses as well as a Commissioner’s signature, the Declarant should bring the required number of witnesses with them.
The Commissioners for Taking Affidavits Act empowers municipal clerks and other officials to take various types of declarations and to administer oaths.
A Commissioner of Oaths is not the same as a Notary Public. The City does not have a Notary Public on staff and therefore we are not able to notarize documents. Please contact a local law office to perform this service.
The Commissioner of Oaths reserves the right to refuse commissioning services for any document.
